Graceville Centre

 

We have supported adults living with mental illnesses, intellectual disabilities and/or physical disabilities throughout the Sunshine Coast for more than 40 years. Our service administration site is located in Nambour.

 

Graceville Centre had a vision to accommodate and support many men and women to help to break the cycle of mental illness. We have a commitment to work with individuals as individuals.”

We work in partnership with people who need support to maintain or expand their current lifestyles. Our clients are: 

  • living in the community
  • transitioning from hospital to the community
  • living with complex needs, and
  • seeking unique assistance.

We aim to assist people to remain or become independent by supporting them to establish and maintain: 

  • health and spiritual wellbeing
  • secure accommodation
  • access to employment, education or volunteering options, and
  • strong relationships within the community.

 

We create an environment that gives individuals the space to explore their current situations and to set and achieve goals. We walk beside and support individuals on their journeys.

 

Support options include: 

  • fully-supported accommodation 24 hours per day, seven days per week
  • transitional shared accommodation
  • semi-supported individual accommodation, and
  • community support for people living independently.

All support options include a comprehensive program aimed at strengthening skills, maximising community involvement and encouraging independence. Our programs are tailored to meet individual needs. We offer support across a broad range of areas to assist clients to:  

  • make a home
  • develop and achieve dreams
  • embrace life
  • become involved and link with others, and
  • be creative.

Participation in all services is voluntary.

 

 

We accept referrals from:

 

•   individuals

•   carers

•   family members

•   medical practitioners, and

•   health care and mental health professionals.
